They have been attending conferences across the world and one of the recent conferences is on “The Newer Modalities of treatment using Dermal substitutes in Plastic Surgery”.
Dr. Rajesh Vasu and Dr. BRN Padmini attended a symposium organized by Matriderm, APAC Symposium at Bangkok, Thailand, on June 26th -27th 2022. The symposium covered wound management and treatment and the latest medical procedure solutions and dermal substitutes in plastic surgery. It’s an option in the armamentarium of plastic surgeons.
Types of wounds:
There are two types of wounds. Acute wounds and Chronic wounds. Acute wounds heal within two to three weeks and sometimes six weeks. They heal with some form of surgical intervention if necessary. Whereas Chronic wounds are the real challenge. They don’t heal even after six weeks. There are various reasons for these types of wounds not healing sooner.
The reasons could be manyfold:
- Vascular problems because of venous ulcers
- Debility or pressure in pressure ulcers
- Host of metabolic and vascular problems in diabetic foot ulcers
So the common goal to treat these wounds is to heal them in the quickest possible time. With the increasing load of chronic ulcers worldwide, there has been a lot of research conducted and a lot of publications. Most of these protocols are based on sound medical evidence and results and proven medical treatment and medicines. There have been path-breaking practices in wound care that changed the way wound heals in the quickest possible time. Thereby reducing the overall cost of managing these chronic wounds.
Two decades ago, there have been path-breaking concepts for managing these difficult wounds. Moist wound dressing and nanocrystalline dressings are for managing the wounds while they are in the healing phase or the initial phase of any deep wound. First is the understanding of wound healing in a better way, where it was found that moist wound healing is the best way to heal a wound. This revolutionized wound treatment. This is followed by the use of nanocrystalline dressing in silver dressings, where by using these, the infection rates and managing the wounds have primarily reduced the time taken for the wounds to heal.
The last decade has seen, the introduction of vacuum-assisted closure in the management of wounds, and this has vastly improved the healing time and lessened hospital stay. Significantly it avoided difficult plastic surgery procedures that use flaps and major microvascular procedures to cover the wounds. This reduced the cost of managing these wounds, lessened hospital stay, and quicker healing of wounds for the patients. In the last couple of years, there has been a lot of talk and research on dermal substitutes for healing these wounds. Dermal substitute is for coverage of any deep wound which is not healing on its own.
Conference Takeaway:
This conference is all about dermal substitutes in plastic surgery. There has been an ample amount of evidence reporting and results from Germany, the UK, and Korea about the effective use of dermal matrices in difficult wounds. So the takeaway is, if used with sound clinical judgment, the healing can be reduced to half the time when compared to using only negative pressure alone.
